Drug Test
1. Urine Testing (Swab/Oral Fluid Testing)

DNA Testing

Notary Service
1. Acknowledgments
This is one of the most common notarial acts.

Please read this disclaimer carefully before utilizing any services provided by Fast Check Labs LLC (referred to herein as "FCL," "we," "us," or "our"). By engaging FCL for Drug Screening or DNA Identity Services, you agree to be bound by the terms of this Disclaimer.
1. Drug Testing and Accuracy Limitations
1.1. Testing Methodology: FCL utilizes advanced laboratory techniques and follows established industry protocols. However, no testing process is 100% infallible. Results are subject to minor variables inherent in sample collection, processing, and analysis.
1.2. False Positives/Negatives: While rare, FCL acknowledges the possibility of false positive or false negative results, which can be caused by various factors including, but not limited to, cross-reactivity with prescribed or over-the-counter medications, adulteration, or biological anomalies.
1.3. Confirmation Required: Preliminary positive results are presumptive only. FCL strongly recommends that all presumptive positive results be confirmed through secondary, highly sensitive methodologies (such as GC/MS or LC-MS/MS) to ensure definitive accuracy.
1.4. Medical Advice: The results of drug or alcohol tests provided by FCL are strictly factual reports on the presence or absence of substances and do not constitute medical advice, diagnosis, or treatment recommendations. Clients must consult with qualified medical professionals for the interpretation of results in a clinical context.
2. DNA Testing and Paternity/Identity Services
2.1. Court-Admissibility: Where requested, FCL will follow proper chain-of-custody procedures necessary to ensure DNA test results are eligible for court submission. However, FCL makes no guarantee that any court, judge, or legal entity will accept, rely on, or rule based on these results.
2.2. Sample Integrity: The accuracy of DNA results is dependent upon the quality and integrity of the collected sample. FCL is not responsible for errors resulting from mislabeled, contaminated, or improperly collected samples provided by the client or third-party collectors.
2.3. Identity Verification: FCL assumes that all parties submitting samples are legally authorized to do so and that the identifying information provided (names, dates of birth, etc.) is accurate. FCL is not liable for errors or disputes arising from identity fraud or misinformation provided by the client.
3. Limitation of Liability
3.1. No Warranty: All services are provided "as is" without any express or implied warranties, including but not limited to warranties of fitness for a particular purpose or merchantability.
3.2. Exclusive Remedy: In the event of a proven error, negligence, or breach of contract by FCL resulting solely in inaccurate test results, the client’s sole and exclusive remedy shall be limited to a re-test, performed at no cost, or a refund of the fees paid for the specific test in question.
3.3. Exclusion of Damages: To the fullest extent permitted by law, FCL shall not be liable for any indirect, special, incidental, consequential, punitive, or exemplary damages, including but not limited to loss of wages, loss of reputation, or emotional distress, arising out of or in connection with the use of our services or the reliance on any test result.
3.4. Maximum Liability: In no event shall the total liability of Fast Check Labs LLC for any claim, loss, or damage arising under this Disclaimer exceed the total amount paid by the client to FCL for the specific service from which the liability arose.
4. Confidentiality and Jurisdiction
4.1. Confidentiality: FCL maintains strict confidentiality protocols regarding test results and personal information, adhering to all applicable privacy laws (e.g., HIPAA where relevant to health information).
4.2. Governing Law: This Disclaimer shall be governed by and construed in accordance with the laws of the State of Missouri, without regard to its conflict of law principles.
